How to apply: Application will be submitted on the prescribed application form provided herewith. The applicant to write clearly the post he is applying for and in which category (SC/ST/OBC/UR), he will appear in the exam.

The application form duly filled in with recent passport size photograph and relevant documents to be addressed to Commanding Officer, 188 Military Hospital, Pin-900188, C/o 99 APO, Likabali Assam). Attach relevant certificate, duly attested, as mentioned at SI No 04 to 08 of Appx. ‘A’ attached along with self addressed empty envelope affixing posted stamp of Rs.25/- for dispatch of call letter. The envelop of application will be superscribed with ‘APPLICATION FOR THE POST OF…….. while sending the application form. The last date of receipt of application is 30 days from the Publication of this advertisement.

Two recent passport size photograph are required of which one photograph pasted on application. Form duly attested by Gazetted Officer. Attested Xerox copies of the following documents are required:

(i) Matriculation or equivalent certificate showing date of birth.
(ii) Mark sheet of matriculation or equivalent certificate.
(iii) Experience certificate if any
(iv) Caste certificate in prescribed format by the Competent authority.
(v) Copy of Aadhar Card/PAN Card.
(vi) Copy of Bonafide certificate
(vii) Certificate of ITI for the post of painter and tailor
